Welcome to the Your Care Network knowledge exchange portal. This site aims to support knowledge sharing related to the Your Care Network programme to test and treat persons living with HIV/A (PLWHA) in the City of Johannesburg. You will be able to locate all resources required to assist general practitioners and support staff in the management of clients and efficient administration of the programme.

The “Your Care Network”, previously known as the “GP Care Cell Programme”, is the pioneer of value-based contracting in primary healthcare. The previous version has been active for more than 2 years in Tshwane and Ekurhuleni. Our objective is to expand access to HIV testing and treatment services at no cost to patients, whilst remunerating healthcare professionals for good clinical outcomes and working with government to develop new models to contract with private sector service providers at scale. Department of Health through the Gauteng Provincial Department of Health funds medicines, test kits and access to laboratory services; USAID and PEPFAR through Anova Health Institute fund clinical fees for practitioners.

Professional Provider Organisation Services (PPO Serve) and the Foundation for Professional Development (FPD), collaborating with the ANOVA Health Institute, contract general practitioners (GP’s) and community pharmacies into the “Your Care Network” to screen for HIV, initiate new patients on first line anti-retroviral treatment (ART) and maintain virally suppressed patients in the City of Johannesburg (CoJ).

This programme is designed for:

  • People who find it difficult to go to public sector clinics for free HIV services
  • People who don’t know their HIV status and would like to take a free HIV test
  • People living with HIV who have never been on treatment before
  • People who do not have medical aid but access health services in the private sector. (Persons with a medical aid must enrol through the appropriate method)
